Weight Management

Local and national weight-management support, from self-help tools to specialist services.

Oviva – Tier 3 Specialist Weight Management

Fully-remote NHS Tier 3 weight-management programme: a personal health coach plus a dietitian-led team, app-based tracking, and prescription weight-loss medication delivered to the door where appropriate. Oviva manages the full journey — coaching, prescribing, monitoring and side-effects — with no GP follow-ups needed. Referral must be made by the GP practice — patients cannot self-refer.

Who it's for & how to access

Who it's for
Adults 18+, assessed as ready and motivated to change (≥7/10 for motivation) and committed to the 12-month programme, AND one of:
• BMI ≥40; or
• BMI ≥35 with a weight-related comorbidity — diabetes, hypertension, CKD, hypercholesterolaemia, ischaemic heart disease, peripheral vascular disease, history of MI/CVA/TIA, liver disease (incl. cirrhosis, NAFLD, gallstones, biliary colic), osteoarthritis or rheumatoid arthritis, back pain, obstructive sleep apnoea, hypothyroidism, Cushing's disease, PCOS, hyperuricaemia or gout, lipoedema, cancer, or asthma; or
• Awaiting bariatric surgery and not already accessed a Tier 3 programme.

Exclusions
Under 18; pregnant or breastfeeding; uncontrolled hypertension, heart condition or any condition preventing increased activity; suicide attempt in the last year; self-harm in the last 3 months; suicide plans in the last 3 months.

Refer only once stable: alcohol or drug misuse, hypothyroidism, Cushing's syndrome.

Post-bariatric-surgery exclusions: less than 2 years post-surgery; unable to tolerate healthy balanced eating / still on a texture-modified (liquid or soft) diet; under T4/bariatric surgery care; current GI pain, vomiting, nausea, recurrent loose stools, dumping syndrome or poor wound healing; symptoms of gut leaking or blockage; not accessing GP support for vitamin/mineral supplementation and monitoring.

How to access / refer
Check eligibility and download the ICB-specific referral form by selecting your ICB at oviva.com/uk/en/refer/#refer. Return the completed form to ovivauk.t3wm@nhs.net, or use the Ardens form on SystmOne/EMIS, or refer via e-RS (Secondary Care → search 'Tier 3 Weight Management Service (medical) Oviva'; Speciality: Endocrinology & Metabolic Medicine; Clinic type: Not otherwise specified; Organisation: Oviva).

Clinical measurements must be in date. Bloods within the last 12 months: LFT, HbA1c, U&E, non-fasting lipids and TSH, plus a BP reading within the last 12 months. Referrals can take up to 15 weeks to process — practice tip: if bloods are more than 8 months old, invite the patient for repeat bloods at referral so results are still valid once it is processed.

Notes
Eligibility can vary by ICB — the referral form and criteria are ICB-specific. Local criteria may add requirements (e.g. a 2-year BMI history, or evidence of previous weight-loss attempts such as Weight Watchers or Slimming World), so check the local form.

NHS Digital Weight Management Programme (DWMP)

Free 12-week online weight-management programme with a choice of providers and three levels of support: digital only; digital plus up to 50 minutes of human coaching; and digital plus up to 100 minutes of tailored coaching. Patients are triaged to a level via the NHS Referral Hub.

Who it's for & how to access

Who it's for
All of: adults 18+; BMI ≥30 (≥27.5 for people from Black, Asian and minority ethnic backgrounds); and a diagnosis of diabetes (Type 1 or Type 2), hypertension, or both. Patient needs a smartphone and an email address.

Exclusions
Any one of: moderate or severe frailty; pregnant; active eating disorder; bariatric surgery in the last 2 years; or where a weight-management programme is judged to pose greater risk of harm than benefit. For patients over 80, refer only where the programme is likely to do more good than harm.

How to access / refer
In general practice, refer via the National e-RS system using NHS England's national referral templates (do not amend them). NHS Midlands & Lancashire CSU e-referral templates can be uploaded into EMIS, SystmOne or Vision — download from the NHS England website; your local data-quality team can help with the upload.

What happens next: the patient receives a text within 2 working days with a link to the Referral Hub, verifies date of birth and ethnicity for triage, then chooses a provider within 6 weeks (or the referral closes). The provider makes contact within 5 working days, and the patient must start within 6 weeks. HCP queries: england.wmp-prevention@nhs.net.

Who it's for
Any adult wanting help to eat more healthily, be more active and lose weight. No referral required.

Self-access online or via the free NHS Weight Loss Plan app.

Specialist Weight Management Service (SaTH)

Specialist multidisciplinary weight management, including pharmacotherapy and assessment for bariatric surgery, delivered by the Shrewsbury & Telford Hospital NHS Trust Centre for Weight Management and Metabolic Surgery.

Who it's for & how to access

Who it's for
Shropshire, Telford & Wrekin adults. BMI ≥40, or ≥35 with a comorbidity (2.5 points lower for people of Black African, African-Caribbean or Asian origin). BMI ≥30 if newly diagnosed with Type 2 diabetes.

How to access / refer
GP referral only.

Diabetes Prevention & Support

Programmes for people at risk of, or living with, diabetes.

Healthier You – NHS Diabetes Prevention Programme (LWTC)

Free behaviour-change programme for adults at risk of Type 2 diabetes — group, digital or 1:1 coaching over 9 months, covering healthy eating, activity and weight loss.

Who it's for & how to access

Who it's for
18+, not pregnant at referral, with non-diabetic hyperglycaemia (HbA1c 42–47 mmol/mol or FPG 5.5–6.9 mmol/l within the last 12 months), or a history of gestational diabetes.

How to access / refer
GP or self-referral. NDH self-referral (needs HbA1c/FPG result, test date, NHS number): lwtcsupport.co.uk/self-register. GDM: lwtcsupport.co.uk/gdm-form.

NHS Type 2 Diabetes Path to Remission (Counterweight)

Low-calorie total diet replacement for up to 12 weeks plus behavioural support, aiming for >10kg loss, improved blood glucose and reduced diabetes medication.

Who it's for
18–65, Type 2 diabetes diagnosed within the last 6 years, BMI ≥27 (≥25 for Black, Asian and other ethnic groups), HbA1c in range, meets criteria with no exclusions, living in Shropshire.

How to access / refer
Speak to the GP practice to check eligibility; the practice refers to Counterweight.

NHS Healthy Living for Type 2 diabetes

Free online NHS programme to help people live well with Type 2 diabetes — self-paced information and advice, usable alongside other diabetes support.

18+, living with Type 2 diabetes (or caring for someone who is).

Structured diabetes education (X-Pert / DAFNE)

Free structured education for people living with diabetes — X-Pert (face-to-face) for Type 2, DAFNE for Type 1.

Type 2: 19+ with a Type 2 diagnosis (not pre-diabetes). Type 1: DAFNE, via GP referral only.

How to access / refer
Book a first appointment via the online booking form (choose a venue and time). Patients registered at Clayton, Ellesmere, Bishops Castle, Cleobury Mortimer, Highley or Severn Fields can also book directly through their practice's in-house stop-smoking clinic. Notes
Prescription-only medicines (varenicline, bupropion/Zyban, cytisine) are outside the service. Specific support is available for pregnant smokers — see the Council's stopping-smoking page. Telford & Wrekin residents should use the T&W Healthy Lifestyles Service instead.

Support for children, young people and families — emotional wellbeing, activities and family help.

CAMHS Shropshire, Telford & Wrekin (CAMHS STW)

NHS mental health service for children and young people aged 0–25 in Shropshire, Telford & Wrekin, provided by MPFT — the new CAMHS STW model launched on 1 April 2026 (previously BeeU). Supports feelings, behaviour and mental health, with a single 'front door' for information and referrals, earlier help and support while waiting, and joined-up care. Includes the ADHD and autism (neurodevelopmental) assessment pathway, the Community Eating Disorder Service, crisis support and school-based Mental Health Support Teams.

Who it's for
Children and young people aged 0–25. Planning for any move to adult services begins around age 16, with support able to continue to 25 and quick routes back in for six months after discharge.

How to access / refer
One 'front door' for all children and young people's mental health referrals — via GP, school, or the single point of access, where a triage team directs to the right support. If a child is already receiving support, their care continues under CAMHS STW with no need to re-refer. Community Eating Disorder Service (0–18)

Part of CAMHS STW — community-based support for children and young people aged 0–18 with eating difficulties and eating disorders, including early help, medical monitoring, family-based support with plans for home and school, and new Day Hospital and Intensive Support at Home models of care.

Mental Health Support Teams (in schools)

School-based early intervention for mild to moderate mental health issues (such as anxiety, low mood and friendship problems) for children and young people aged 5–16/18 — evidence-based 1:1 and group support (e.g. CBT), plus help for school staff and parents/carers. Expanding to all STW schools by 2030.

Diagnosis and memory support, and help for people living with dementia and their families.

Before your appointment, note what you've already tried to lose weight and how it went, any health conditions linked to your weight (like high blood pressure or Type 2 diabetes), and why you'd like support now. You can ask whether you're eligible for a Tier 3 service such as Oviva.
